Medical and Lifestyle Special diet form ​

All children in Reception, Year 1 and Year 2 are entitled to Universal Infant Free School Meals.

​

From September 2026, all pupils from households in receipt of Universal Credit will be entitled to receive a free meal.

There are two categories of benefits-related free school meals:

1. Targeted Free School Meals (TFSM) – children and young people in families receiving Universal Credit with annual household income under £7,400 (£616.67/month)

2. Expanded Free School Meals (EFSM), a new category – children and young people in families receiving Universal Credit with no limit on annual household income

You will need to provide the school with:

• your full name

• date of birth

• and National Insurance (NI) number or National Asylum Support Service (NASS) reference number

 

We will then share your details with the City of Wolverhampton Council who check eligibility using a secure Government website on behalf of the school. If you are found eligible, we will let you know.

 

If you have No Recourse to Public Funds (NRPF), you will need to complete an application form and provide additional evidence to the school.

Schools receive valuable Government funding (Pupil Premium) for every child in receipt of Targeted FSM.

Eligibility for FSM also entitles children to access the Holiday Activities and Food (HAF) Programme during Christmas, Easter and Summer school holidays
